Le diagnostic de s\u00e9cheresse va \u00eatre confirm\u00e9 par un test d\u2019impr\u00e9gnation des larmes sur 5 min ou mesure du flux salivaire. Le diagnostic de Sjo va \u00eatre confirm\u00e9 par le bilan bio (pr\u00e9sence d\u2019auto-anticorps anti SSA et\/ou biopsie des glandes salivaires accessoires (cf vid\u00e9o FAI2R).<\/span><br \/>\n<span style=\"font-size: 10pt; font-family: helvetica;\"><strong><span style=\"color: #ffffff; background-color: #800080;\">Polyarthrite rhumato\u00efde (PR)\u00a0<\/span><\/strong> : maladie auto-immune se r\u00e9v\u00e9lant par des arthrites (gonflements) des mains, des pieds et \u00e9galement les grosses articulations et se caract\u00e9risant par la pr\u00e9sence d\u2019anticorps anti-CCP sp\u00e9cifiques de la maladie (dans 75 % des cas) et par la pr\u00e9sence de facteurs rhumato\u00efdes. <\/span><br \/>\n<span style=\"font-size: 10pt; font-family: helvetica;\"><strong><span style=\"background-color: #800080; color: #ffffff;\">Lupus \u00e9ryth\u00e9mateux syst\u00e9mique\u00a0<\/span><\/strong> : maladie auto-immune syst\u00e9mique c\u2019est \u00e0 dire qui peut potentiellement atteindre la plupart des organes. Les atteintes les plus fr\u00e9quentes sont les atteintes articulaires et cutan\u00e9es. Il y a autant de formes cliniques que de patients. La maladie touche plus fr\u00e9quemment les femmes que les hommes avec un \u00e2ge de d\u00e9but moyen autour de 25-30 ans. <\/span><br \/>\n<span style=\"font-size: 10pt; font-family: helvetica;\"><strong><span style=\"color: #ffffff; background-color: #800080;\">Pseudopolyarthrite rhizom\u00e9lique\u00a0<\/span><\/strong> : La pseudo polyarthrite rhizom\u00e9lique (PPR) est un rhumatisme inflammatoire chronique caract\u00e9ris\u00e9 par un enraidissement douloureux des \u00e9paules et du pelvis associ\u00e9 \u00e0 une alt\u00e9ration de l\u2019\u00e9tat g\u00e9n\u00e9ral et \u00e0 un syndrome inflammatoire biologique. Elle survient chez les sujets \u00e2g\u00e9s de plus de 50 ans. La PPR peut \u00eatre associ\u00e9e \u00e0 une inflammation des gros vaisseaux appel\u00e9e maladie de Horton ou art\u00e9rite \u00e0 cellule g\u00e9ante. <\/span><br \/>\n<span style=\"font-size: 10pt; font-family: helvetica;\"><strong><span style=\"color: #ffffff; background-color: #800080;\">SPA (spondyloarthrite) <\/span><\/strong>: rhumatisme inflammatoire chronique. Elle est li\u00e9e \u00e0 l\u2019inflammation des enth\u00e8ses, c\u2019est-\u00e0-dire des zones d\u2019insertion sur l\u2019os des tendons, ligaments, capsule articulaire. La SpA se manifeste par des douleurs de rythme inflammatoire qui touchent la colonne vert\u00e9brale, le bassin au niveau des articulations sacro-iliaques mais aussi les articulations p\u00e9riph\u00e9riques. Elle peut \u00eatre associ\u00e9e \u00e0 des maladies extra rhumatologiques comme les MICI (Maladies Inflammatoires Chroniques Intestinales comme la maladie de Crohn et la rectocolite h\u00e9morragique) ou le psoriasis. <\/span><br \/>\n<span style=\"font-size: 10pt; font-family: helvetica;\"><strong><span style=\"color: #ffffff; background-color: #800080;\">Goutte\u00a0<\/span><\/strong> : il s\u2019agit du plus fr\u00e9quent des rhumatismes inflammatoires. La goutte est li\u00e9e \u00e0 des d\u00e9p\u00f4ts de cristaux d\u2019urate sodium, dans les articulations, les tendons, et aussi la peau, les reins. C\u2019est une maladie chronique qui \u00e9volue par crises d\u2019arthrites aigu\u00ebs r\u00e9cidivantes, tr\u00e8s souvent de la premi\u00e8re articulation m\u00e9tatarso-phalangienne (gros orteil), ou le pied, la cheville, le genou.<\/span><br \/>\n<span style=\"font-size: 10pt; font-family: helvetica;\"><strong><span style=\"background-color: #800080; color: #ffffff;\">Rhumatisme \u00e0 pyrophosphate de calcium (Chondrocalcinose)\u00a0<\/span><\/strong> : comme la goutte il s\u2019agit d\u2019un rhumatisme inflammatoire li\u00e9 \u00e0 des d\u00e9p\u00f4ts de microcristaux. La chondrocalcinose est li\u00e9e \u00e0 des d\u00e9p\u00f4ts de pyrophosphate de calcium au niveau des cartilages et fibrocartilages. Pr\u00e9dominante chez la femme, la chondrocalcinose est une maladie du sujet \u00e2g\u00e9, dont la fr\u00e9quence augmente avec l\u2019\u00e2ge. C\u2019est une maladie qui \u00e9volue par crises intermittentes.<\/span><br \/>\n<span style=\"font-size: 10pt; font-family: helvetica;\"><strong><span style=\"color: #ffffff; background-color: #800080;\">Scl\u00e9rodermie <\/span><\/strong>: maladie auto-immune syst\u00e9mique, c\u2019est-\u00e0-dire qu\u2019elle peut atteindre diff\u00e9rents organes notamment la peau, les articulations, le c\u0153ur, les poumons, le syst\u00e8me digestif, les reins. Il s\u2019agit d\u2019une maladie rare. Son atteinte caract\u00e9ristique est une scl\u00e9rose de la peau, qui lui donne son nom de scl\u00e9rodermie.<\/span><\/p>\n<p><span style=\"font-size: 14pt; font-family: helvetica;\"><strong><span style=\"color: #000080;\">8.\u00a0<span style=\"text-decoration: underline;\">Pr\u00e9sentation des gestes sp\u00e9cifiques au service :<\/span><\/span><\/strong><\/span><br \/>\n<span style=\"font-family: helvetica;\"><span style=\"font-size: 10pt;\"><strong><span style=\"background-color: #3366ff; color: #ffffff;\">Infiltrations \u00e9pidurales par voie lombaire et par la voie du hiatus (sacro-coccygien)\u00a0<\/span><\/strong> : L\u2019infiltration \u00e9pidurale consiste \u00e0 injecter un m\u00e9dicament anti-inflammatoire \u00e0 base de cortisone dans l\u2019espace \u00e9pidural (dit aussi \u00ab p\u00e9ridural \u00bb). Sa principale indication est le traitement de la douleur en cas de lombosciatique unilat\u00e9rale radiculaire m\u00e9canique lorsque d\u2019autres traitements m\u00e9dicaux ont \u00e9chou\u00e9. Elle peut se faire dans le bas du dos (voie haute) ou au niveau du hiatus sacro-coccygien entre le sacrum et le coccyx.<br \/>\n<\/span><span style=\"font-size: 10pt;\"><strong><span style=\"background-color: #3366ff; color: #ffffff;\">Flux salivaire \/ Test de Schirmer\u00a0<\/span><\/strong> : ces examens permettent d\u2019\u00e9valuer s\u2019il existe une s\u00e8cheresse objective au niveau de la bouche (flux salivaire) ou des yeux. Pour le flux salivaire, on mesure la production de salive en 5 mn. Pour le test de Schirmer : des petites bandelettes de papier buvard sont plac\u00e9es pendant 5 minutes entre la paupi\u00e8re inf\u00e9rieure et la corn\u00e9e pour mesurer la quantit\u00e9 de larmes.<br \/>\n<\/span><span style=\"font-size: 10pt;\"><strong><span style=\"color: #ffffff; background-color: #3366ff;\">Ponction articulaire\u00a0<\/span><\/strong> : La ponction d&rsquo;une articulation gonfl\u00e9e et douloureuse permet d\u2018\u00e9vacuer le liquide articulaire en exc\u00e8s. Il s\u2019agit d\u2019aspirer le liquide fabriqu\u00e9 en surplus avec une seringue apr\u00e8s avoir plac\u00e9 une aiguille dans l\u2019articulation. Ce geste permet de soulager la douleur, et aussi d\u2019analyser le liquide articulaire (tubes st\u00e9riles bouchon rouge pour la Bact\u00e9rio et tubes bouchon vert \u00e0 envoyer en anapath + 1 tube bouchon vert en Bact\u00e9rio), pour mieux comprendre la cause du gonflement et notamment de v\u00e9rifier qu\u2019il n\u2019existe pas d\u2019infection. Elle se fait au lit du patient. Une anesth\u00e9sie locale par patch de lidoca\u00efne peut \u00eatre r\u00e9alis\u00e9e avant le geste.<br \/>\n<\/span><span style=\"font-size: 10pt;\"><strong><span style=\"color: #ffffff; background-color: #3366ff;\">Echographie articulaire \/geste sous \u00e9cho <\/span><\/strong>: l\u2019\u00e9chographie articulaire est devenue l\u2019outil indispensable au rhumatologue jeune comme plus vieux pour mettre en \u00e9vidence un \u00e9panchement, ponctionner sous \u00e9chographie. Elle permet aussi de voir si la polyarthrite est encore active c\u00e0d s\u2019il existe encore une inflammation.<br \/>\n<\/span><span style=\"font-size: 10pt;\"><strong><span style=\"color: #ffffff; background-color: #3366ff;\">Echographie des glandes salivaires\u00a0<\/span><\/strong> : cet examen pratiqu\u00e9 par certains rhumatologues du service avec une vacation d\u00e9di\u00e9e le jeudi matin lors de l\u2019HDJ Syndromes secs permet de rechercher des anomalies typiques de la maladie de Sj\u00f6gren.<br \/>\n<\/span><span style=\"font-size: 10pt;\"><span style=\"color: #ffffff; background-color: #3366ff;\"><strong>Biopsie des glandes salivaires (BGSA)<\/strong> <\/span>: lors d\u2019une courte anesth\u00e9sie locale d\u2019une partie de la l\u00e8vre inf\u00e9rieure, apr\u00e8s une l\u00e9g\u00e8re incision, on pr\u00e9l\u00e8ve quelques glandes salivaires accessoires. Ce geste est peu invasif, pratiqu\u00e9 au lit du patient ou en consultation.
